Welcome to California Hematology Oncology Medical Group!  The following steps will guide you to becoming a new patient at our office.

Step 1: Contact our New Patient Coordinator, Corene Morita, at: (310) 649-7222, to schedule an appointment. 

Step 2: Print the New Patient Packet.  Requires Adobe.  Please bring this packet with you to your appointment.  Once you have printed this form, you may need to hit the "BACK" button on your browser to return to this page.If do not have Acrobat Reader, you may download this software here. 

Step 3: Fill out the Online Patient Information Form and your information will be submitted electronically, via a secure server.  This form takes approximately 5-10 minutes to complete.

Approximately 1 day prior to your appointment, a staff member will call you prior to as a courtesy reminder.
